What documents does a foreigner need for a Thai driving licence?

Short answer

The Department of Land Transport generally asks for the passport with a valid permission to stay, evidence of residence in Thailand (a certificate of residence from Immigration or from your embassy, or an accepted alternative), and a recent medical certificate — followed by the tests the office requires for the category applied for.

Holders of a valid licence from another country are often examined more lightly, but the treatment depends on the licence, the country and the office, and a translation of the foreign licence may be required. An International Driving Permit is a travel document, not a substitute for a Thai licence once you are resident, and it is issued against a home-country licence that must remain valid.

Two practical points: the medical certificate has a short shelf life, so obtain it in the days immediately before the appointment, and the residence evidence has its own validity period set by the issuer.
